The company was cited twice for both not offering proper safety training on how to operate the aerial bucket as well as for having an employee operating the machine while not being trained. Each violation cost the company $5,000 for fines totaling $10,000.

It wasn’t the only incident for the construction company this year; it wasn’t even the only major work accident in that month. Another employee of the company was killed in an Indianapolis work accident when he was caught beneath a 40,000lb excavator.
