Accutane (isotretinoin), made by Hoffmann-LaRoche, entered the market in 1982 to treat only the most severe forms of acne that could not be treated with other medication. Since then, more than five million people in the United States and 12 million people around the world have used Accutane. Some have suffered severe side effects. The most widely publicized adverse reactions to Accutane have been suicide, depression, severe birth defects, and damage to the liver, kidneys, and colon.

The United States Food and Drug Administration (FDA) first required Hoffmann-LaRoche to increase warnings about the danger of birth defects as a result of using Accutane in 1983. In 2005, the FDA approved a strengthened risk management plan for Accutane to make sure women did not become pregnant while taking the medicine because it caused severe birth defects as well as miscarriages. That same year, the FDA also approved new warnings for risks of depression and psychiatric problems. More recently, Accutane has been linked to incidents of severe pancreatitis and inflammatory bowel disease, sometimes requiring surgical removal of certain organs.

In June 2009, Hoffmann-LaRoche announced that it was removing the drug from the U.S. market stating increased competition of generics and the cost of defending itself from lawsuits. So far, the total amount of lawsuit awards to Accutane users is $56 million. The drug is so dangerous that the FDA changed its rules so that doctors must now get patients to sign a consent form saying that the patient understands all the risks associated with Accutane. It is also one of just three drugs in the United States that has ever been required to come with a Medguide (a detailed warning distributed by pharmacists).
